You'll enter your modem's external IP address (not the internal).

Yes.

Although you can pre-populate your name, password and domain, I suggest
you leave these blank. Filling in these fields is simply a time saver
for connecting.
Question for you: The Mac and Windows machines are on different networks
(such as a home and work network) and not on the same network (both at
home or both at work), right? And you're attempting to connect the Mac
to the Windows machine across the Internet?
