Man, I work in that field and I have no clue than maybe to clear font
caches 
(FontNuke).  But I would narrow down the culprit:
- Same font, different apps at large size
- Different OFT fonts, same apps, large sizes
- a PDF made using Apple driver
- a PDF made using Distiller
- print to a different printer (non-PS or non-Adobe ... but you would
think 
Adobe PS is the right way to go.
