Weird.

What happens when you enable SuperSleek or Sleek-A-Boo? Does that give 
you at least both the the Sleek X-Headers panes (and perhaps somehow 
magically :) the Extra Headers one as well)?

Btw, since both SuperSleek and Sleek-A-Boo seem to have disappeared from 
the 'net and people often ask about it, I've put Sleek-A-Boo on-line:
